Lidocaine 2%
Mechanism of Action
Suppresses ventricular arrhythmia by decreasing automaticity.  Some studies
have shown that lidocaine can elevate the cardiac fibrillation threshold,
reducing the chances of developing V-fib

Indications
Initial antiarrhythmic for the treatment of pulseless V-tach and/or V-fibrillation
Treatment of symptomatic wide-complex tachycardias
Treatment of symptomatic ventricular ectopy, i.e. PVCs

Onset & Duration
Onset: 30-90 sec
Duration: 2-4 hours

Doses and Administration
In the arrest scenerio, IV Push: Give 1-1.5 mg/kg, total bolus 3.0 mg/kg
Endotracheal tube:
First dose – 200 mg
Second dose – 100 mg diluted in 5 ml of NaCl
Maximum loading dose is 3.0 mg/kg
For suppression of ventricular ectopy
First dose: 1-1.5 mg/kg
Subsequent doses: .5-.75 mg/kg
Total loading dose 3.0 mg/kg

Pediatrics
For ventricular fibrillation refractory to epinephrine: 1mg/kg
