Lidocaine 1gm in 250 D5w
Mechanism of Action
Lidocaine Infusion is used to maintain theraputic levels after
suppression of ectopy by a lidocaine 2% bolus. It suppresses
ventricular arrhythmia by decreasing automaticity.  Some studies
have shown that lidocaine can elevate the cardiac fibrillation
threshold, reducing the chances of developing V-fib.

Indications
Used as a IV drip to maintain theraputic levels.
Given after Lidocaine bolus has supressed ventricular ectopy.
Given after resuscitation in the arrest scenerio in which
Lidocaine bolus resulted in conversion

Packaging
1 Gm in 250 ml D5W (pre-pack)
Drug box inventory = 1

Onset & Duration
Onset: 30-90 sec
Duration: As long as IV infusion occurs

Doses and Administration
IV infusion 2-4 mg/min
With the provided concentration and mini-drip (60 gtt/ml) tubing
use a chart to administer Lidocaine by infusion:

Steps to calculate proper doses:
Determine total loading bolus of Lidocaine 2%
Add 1 to total bolus for desired dose
Input desired dose into chart for approximate drip rate.
Set drip rate, and monitor patient

Precautions
In persons with left ventricular dysfunction may cause myocardial
depression
Large doses may induce heart blocks, or alter AV conduction
Excessive doses can cause:
Myocardial depression
Circulatory depression
Neurological depression, to include:
Drowsiness
Disorientation
Decreased hearing
Paresthesia
Muscle twitching
Focal or grand mal siezures

Pediatrics
Rarely used in pediatrics, contact medical control for
consideration
