Nitroglycerine
Mechanism of Action
Nitroglycerine provides vascular dilation by binding to specific receptors and relaxing
the smooth vascular muscles. This reduces preload and to a lesser extent afterload,
decreasing myocardial oxygen demand. Nitroglycerin has also been shown to
increase perfusion in the coronary arteries and collateral blood flow to ischemic
myocardium.

Indications
Angina pectoris (either exertional or resting)
Unstable Angina pectoris
Acute myocardial infarction
First drug in the treatment of severe pulmonary edema

Packaging
0.4 mg tablets (multi-dose bottle)

Onset & Duration
Onset: 1-3 minutes
Duration: 20-30 minutes

Doses and Administration
First dose: 0.4 mg tablet sub-lingual
If no effect in 3-5 min, 0.4 mg tablet sub-lingual
If no effect in 3-5 min, 0.4 mg tablet sub-lingual
With Medical Control's permission, can be administered by the EMT-B, if patient
already has a Nitro perscription

Pediatrics
No indication for use in pediatric patients

Precautions
Blood pressure must be monitored carefully
Should be used with caution (by paramedics only) in the patient taking Viagra
May cause:
Transient headache
Postural syncope
Reflex tachycardia
Hypotension
Nausea and vomiting
Allergic reaction
Muscle twitching
Diaphoresis
